National health data, in the form of rates of disease, is based on the Kaiser Family Foundation’s State Health Facts, which uses the Kaiser Commission on Medicaid and the Uninsured (KCMU) analysis of the Centers for Disease Control and Prevention (CDC)'s Behavioral Risk Factor Surveillance System (BRFSS) 2014 Survey Results.
